P219A on a Cadillac is "Bank 1 Air-Fuel Ratio Imbalance." Instead of the whole bank running rich or lean, the engine computer has spotted one cylinder on bank 1 burning a different mixture from its neighbors. It is a newer, subtler code than P0171 or P0172, and on Cadillacs GM has documented two causes most generic pages never mention: on the 3.6L V6 in the CTS, ATS, CT6, XT5 and XT6 — plus the CT5 and CT6 twin-turbos — an Active Fuel Management rocker arm that stops opening its valve; and on Cadillac V8s, a replacement injector of the wrong flow class.

What Does P219A Mean on a Cadillac?
Fuel trim codes such as P0171 and P0172 tell you a whole bank is off. P219A is different: the bank's average mixture may be fine, but the cylinders within it are not equal. One cylinder is getting too much fuel or too little air — or too little air because a valve isn't opening — and the others are compensating.
The ECM can't measure each cylinder's mixture directly, so it watches how much the upstream oxygen sensor signal fluctuates as each cylinder's exhaust pulse passes. A balanced bank produces a steady signal; an imbalanced one produces a ripple that repeats once per engine cycle. P219A sets when that ripple, compared with a known-good engine at the same speed and load, is too large.
P219A is a symptom of one cylinder being different, not a part. Replacing the oxygen sensor that detected the ripple rarely fixes it. For the general GM background on this code, see our P219A GM guide. This page covers what GM has documented for Cadillac models.
Which Cadillacs Set P219A
GM's own documents name P219A on these Cadillacs:
- 3.6L V6 (LGX): 2016–2019 ATS and CTS, 2016–2020 CT6, 2017–2021 XT5, 2020–2021 XT6 (PIP5802).
- 3.0L twin-turbo V6: 2016–2020 CT6 (LGW) and 2020–2021 CT5 (LGY) (PIP5802).
- V8s: 2014–2020 GM V8 gasoline engines including Cadillac's 6.2L L86 (Escalade), LT4 (CTS-V) and the CT6-V's LTA (20-NA-098).
Where bank 1 is:
- Longitudinal 3.6L and 3.0TT (ATS, CTS, CT5, CT6): bank 1 is the bank with cylinder 1 — the passenger side — holding cylinders 1, 3 and 5.
- Transverse 3.6L (XT5, XT6): bank 1 is the rear bank, nearest the firewall; GM's transverse V6 documents call the front bank "bank 2."
- Small-block V8 (Escalade, CTS-V): bank 1 is the driver's side — cylinders 1, 3, 5 and 7.
- Four-cylinder Cadillacs: the whole engine is bank 1. The bank 2 version of this code, P219B, exists only on V6 and V8 engines.

The AFM Rocker Arm Bulletin
GM preliminary information PIP5802 (April 2021) is a diagnostic tip for P219A or P219B setting with no other driveability symptoms. It covers the Cadillac ATS, CTS, CT6, XT5 and XT6 with the 3.6L LGX, the CT6 with the 3.0L LGW and the CT5 with the 3.0L LGY, along with Buick, Chevrolet and GMC models sharing those engines.
GM's finding: after normal service-information diagnosis finds nothing, there may be an Active Fuel Management (AFM) rocker arm that isn't opening the valve, because of:
- a broken locking pin shelf, or
- a stuck locking pin.
On these engines, AFM shuts off two cylinders at light load by letting special switchable rocker arms (GM calls them SRFFs — switching roller finger followers) collapse instead of opening the valves. If a locking pin breaks or sticks, the rocker can stay collapsed when it should be working. That cylinder then breathes poorly or not at all on one valve, its mixture goes off, and the oxygen sensor sees the imbalance.
GM's test is mechanical: remove the camshaft cover over the AFM cylinder and turn the engine over by hand while watching the rockers and valves on cylinder 2 or 5. The rocker should open the valve, not merely move with the camshaft. If an intake or exhaust valve doesn't open, that is the imbalance. GM recommends replacing both rockers on that camshaft — intake or exhaust — and then re-evaluating.
By GM's cylinder numbering, cylinder 5 is on bank 1 and cylinder 2 is on bank 2 — so on these engines a P219A points at cylinder 5's rockers, and P219B at cylinder 2's. GM notes it had asked for this information to be added to the service-information diagnostics for these codes.
Injector Flow Class on V8s
GM bulletin 20-NA-098 (May 2020) covers 2014–2020 GM V8 gasoline engines — for Cadillac, the L86 Escalade, LT4 CTS-V and LTA CT6-V. Its subject lists P219A and P219B along with misfire and fuel trim codes.
The reason is simple once you know how GM builds fuel rails. At the supplier, each rail is fitted with a mix of nominal, low and high flow injectors to balance flow across the rail for emissions. If a technician replaces one injector with the wrong class, that cylinder now gets more or less fuel than its neighbors — the definition of an air-fuel imbalance.
GM's instruction:
- Read the eight-digit part number on the injector body.
- Enter the VIN in the parts catalog, find that number in the fuel injector rail section, and order the service part listed beside it.
- If the injectors have been removed or mixed up, order the "NOM FLOW" injector in the quantity needed.
GM also notes replacement injector kits carry different part numbers from the injectors inside them. If your Escalade or CTS-V set P219A after injector work, this is the first thing to check. Separately, GM's PIP5891D investigated injector-caused misfires on the 2022–2023 Escalade 6.2L L87; it names injector-related codes, not P219A, but it shows injectors are a live concern on that engine.
How GM Detects It
GM's 2018 OBD summary tables for an engine group that includes the CT6's 3.0L LGW, the CTS-V's LT4, the 2.0L LTG and the XTS's 3.6L LFX describe the P219A monitor in detail. The ECM samples the pre-catalyst oxygen sensor voltage once per cylinder event, calculates its variance, subtracts the variance expected from a good system at that speed and load, divides by a normalizer and applies a quality factor — all from 17×17 calibration tables. The result is a filtered ratio.
- The code fails when the filtered ratio stays above 0.36; after a failure it must fall below 0.25 to pass again.
- The test runs between about 1,700 and 4,279 rpm with steady speed and airflow, fuel level above 10%, and no fuel level sensor fault.
- In that table it is a Type A, one-trip code — one failed trip can light the check engine lamp.
- It needs steady-state driving to collect data, so it may not run on short, stop-start trips.
Thresholds vary by engine and model year; those figures are for that 2018 calibration group.
Symptoms of P219A
- Check engine light, often with no driveability complaint (the PIP5802 case)
- Slight roughness at idle or under light load
- Misfire codes on the same bank in some cases
- Code appearing soon after injector or valvetrain work
Causes and How to Confirm Each
| Cause | How to confirm it | Notes |
|---|---|---|
| AFM rocker arm not opening the valve | Cam cover off, turn engine by hand, watch cylinder 5 (bank 1) rockers and valves (PIP5802) | 3.6L LGX, 3.0L LGW/LGY; replace both rockers on that camshaft |
| Wrong-flow-class injector | Injector body numbers vs parts catalog by VIN (20-NA-098) | Cadillac V8s after injector work |
| Leaking or restricted injector | Injector balance test; injector and misfire codes | Leading suspect on direct injection |
| Intake leak at one runner | Smoke test; inspect the gasket at one port | Leans out one cylinder |
| Low compression or valve problem | Compression and leak-down tests | Also covers a burnt or sticking valve |
| Exhaust leak ahead of the upstream sensor | Inspect manifold and gasket | Can distort the sensor signal |
| Upstream sensor fault | Sensor codes, response check | Less common; the sensor is usually just the messenger |

How Is P219A Diagnosed?
- Record all codes and freeze frame. Note any misfire, injector, fuel trim or O2 codes and fix circuit faults first.
- Check P219B. Both codes together suggest something shared; P219A alone points at bank 1.
- Ask about recent work. Injector replacement on a V8 points at 20-NA-098; valvetrain work points at installation errors.
- Look at misfire counters for bank 1 cylinders to find the odd one out.
- Run an injector balance test and check injector body numbers against the correct service parts.
- Check compression on the suspect cylinder.
- On a 3.6L or 3.0TT with no symptoms, follow GM's PIP5802 rocker check on cylinder 5 if service-information diagnosis finds nothing.
- Smoke test the intake for a single-runner leak.
- Clear codes and drive at steady speeds so the imbalance monitor can run and pass.
Parts and Cost
| Repair | What it involves | Cost picture |
|---|---|---|
| Correct-class injector | Replace the wrong-class injector with the VIN-matched part (20-NA-098) | Moderate on direct injection |
| AFM rocker arms | Replace both rockers on the affected camshaft (PIP5802) | Moderate; camshaft cover removal adds labor |
| Injector replacement for a leak | After a balance test confirms it | Moderate to large |
| Intake gasket | Where a smoke test finds a runner leak | Modest to moderate |
| Valve or compression repair | Where compression testing finds the fault | Large |
| Upstream O2 sensor fitted for P219A | Only if the sensor itself tests bad | Often money spent with the cause still present |

What Other P219A Pages Get Wrong
- "Replace the upstream O2 sensor." The sensor is the messenger. GM's Cadillac documents point at injectors and valvetrain, not sensors.
- Treating it like P0171 or P0172. The bank average can be normal; the problem is one cylinder.
- Ignoring the valvetrain. GM's PIP5802 shows a mechanical AFM rocker fault can set P219A with no other symptoms.
- Replacing one injector with "any" injector. GM's 20-NA-098 says the flow class must match by body number and VIN.
- Putting bank 1 on the wrong side. Passenger side on a CTS or CT6, rear bank on an XT5, driver's side on an Escalade.
- Expecting it to reset quickly. The monitor needs steady driving above about 1,700 rpm on the 2018 calibrations we read.
Frequently Asked Questions
Is P219A serious?
It is usually not an emergency, but one cylinder running off-mixture can lead to misfire, catalyst damage and, with a failed AFM rocker, further valvetrain wear. Diagnose it rather than clearing it.
Can I drive with P219A?
If the engine runs smoothly and the check engine light isn't flashing, usually yes for a short time. A flashing light means active misfire — stop and diagnose.
Why did P219A appear after injector replacement on my Escalade?
GM's 20-NA-098 describes exactly that: the replacement injector may be the wrong flow class for that position. The fix is the VIN-matched part based on the original injector's body number.
What is the AFM rocker problem on the 3.6L?
GM's PIP5802 says a broken or stuck locking pin in an Active Fuel Management rocker on cylinder 2 or 5 can stop a valve from opening. On bank 1 that is cylinder 5. GM replaces both rockers on that camshaft.
What's the difference between P219A and P219B?
P219A is bank 1, P219B is bank 2. Only V6 and V8 Cadillacs have a bank 2.
Will a fuel injector cleaner fix P219A?
It may help a mildly restricted injector, but it won't fix a wrong-class injector, a failed AFM rocker or a compression problem. Find the odd cylinder first.
